BSA Medical Form (Parts A, B & C)
Required for all participants attending events lasting 72+ hours. Part C requires a physician’s signature and is valid for one year.
Pre-Camp Swim Test Form
Complete the swim classification test before arriving at camp to save time during check-in. Scouts must pass to participate in aquatic activities.
Merit Badge Prerequisite Form
Document completed prerequisites for merit badges before arriving at camp. Submit to merit badge counselors during the first class session.
Preparing for the Swim Classification Test
All campers are classified as Swimmer, Beginner, or Learner before participating in any aquatics. We strongly recommend completing the test before camp using the form above — it saves significant time during Sunday check-in. Pre-camp tests must be administered by a certified aquatics professional (BSA Aquatics Instructor, BSA Lifeguard, Red Cross lifeguard, swim instructor, or swim coach).
Swimmer (full aquatics access): Jump feet first into water over your head in depth, level off, and swim 75 yards using a strong stroke (sidestroke, breaststroke, trudgeon, or crawl). Then swim 25 yards using an easy resting backstroke. The 100 yards must be swum continuously and include at least one sharp turn. After completing the swim, rest by floating.
Beginner (restricted to shallow areas): Jump feet first into water over your head in depth, level off, swim 25 feet on the surface, stop, turn sharply, resume swimming, and return to the starting place.
Learner (does not swim): No test required. Learners are restricted to wading areas with supervision.
Common reasons Scouts do not pass: underwater swimming, dog paddling, resting mid-test, treading water without forward motion, not floating at the end, or swimming too fast and appearing exhausted. Practice swimming 100 yards continuously at your local pool before camp.
Activity Waivers
Hold harmless agreements required for specific high-adventure and specialty activities. These are signed at camp during check-in.
COPE & Climbing
Required for all participants in the COPE high-adventure course and climbing tower activities.
Personal Watercraft
Required for all participants using personal watercraft, jet skis, or motorized boating equipment.
Shooting Sports
Required for all participants in rifle, shotgun, and archery programs at camp.
